Emphysema of the lungs is a pathological condition that often occurs with a variety of bronchopulmonary diseases and occupies an extremely important place in pulmonology. Only in a relatively small part of patients is it an independent disease - either genetically determined or due to a defect of development. In clinical practice, it is extremely important to conduct differential diagnosis between primary and secondary pulmonary emphysema, because this determines the features of prevention and therapeutic tactics, as well as a prognosis for this disease. In this case, a variety of clinical, functional, radiological and instrumental methods should be used, the detailed description of which is given in the leadership. The most important complications of pulmonary emphysemes are described, namely, pulmonary hypertension, respiratory failure, spontaneous pneumothorax. Features of the prevention and treatment of pulmonary emphysemes, as well as its complications, are presented. The management is intended for therapists, pulmonologists, phthisiers and radiologists
